The DisplayApp class isn't used in the Screen base class and most
screens, so requiring it is pointless.
In this commit, DisplayApp pointers were added to screens which use it
and the explicit Screen constructor was removed in those screens.
Don't use relative imports like `../foo.h` as those depend on the
relative position of both files. Rather than that use imports relative
to the `src` directory, which explicitly is part of the include
directories.
